Preparing to Receive a Sacrament


The Latin word sacramentum means “a sign of the sacred.” The seven sacraments are sacred celebrations that point to what is holy, significant, and life-giving for Catholics. They are special moments of encounter with God’s saving presence.


In the life of the Church, sacraments are both signs and instruments of God’s grace. They not only point us toward God, but also make His grace truly present and active in our lives.


By learning more about the sacraments, we are able to celebrate them more fully and enter more deeply into the life of the Church.

Sponsor Information & Forms

If you have been asked to serve as a sponsor for Baptism or Confirmation, thank you for accepting this important role in someone’s faith journey. Sponsors are called to be witnesses of the Catholic faith and companions in discipleship.



To serve as a sponsor, certain requirements must be met and appropriate documentation must be completed.

Baptism

Catholic godparents must provide a sponsor certificate from their parish before the Baptism can be scheduled.


If the godparent belongs to Holy Family Parish, they may contact the parish office to request a certificate.


If the godparent belongs to another Catholic parish, they should contact their parish office directly.

Confirmation

Catholic Confirmation sponsors must provide a sponsor certificate from their parish before Confirmation preparation is completed.


If the sponsor belongs to Holy Family Parish, they may contact the parish office at (330) 688-6411 to request a certificate.


If the sponsor belongs to another Catholic parish, they should contact their parish office directly to request and submit the Sponsor Certificate.

Requesting a

Sponsor Certificate


If you are requesting a sponsor certificate, we ask that you contact the parish office personally.


Because serving as a sponsor or godparent involves affirming that you are an active Catholic in good standing with the Church, our staff needs to speak directly with you regarding your request.


While parents and family members often wish to help, sponsor certificates cannot be requested by another person on your behalf. Thank you for your understanding and cooperation.
